'Healthy Denbighshire', the second Health, Social Care & Well-Being Strategy for the entire County is now available online.
The strategy has been developed by the Denbighshire Health, Social Care & Well-Being Partnership and outlines the plans for the next 3 years, from 2008-2011. The aims are to improve health and well-being for the people of Denbighshire, but 'Healthy Denbighshire' is not just about services we might need when we are sick - it is also about what we can do to keep ourselves well.
The policy sets out a strategic approach to help deliver better services to those who need them and to support individuals and communities to improve their own health and well being.
A campaign has been launched by the Welsh Assembly Government to help the public choose the most appropriate health service to their needs. The campaign is designed to lower unnecessary A&E attendances and prevent incorrect use of the 999 emergency service as well as helping patients make better informed decisions about their own care.
As part of this, a "Choose Well” thermometer is available with a range of colours for different conditions, which acts as an easy guide to showing the service most appropriate to each illness.
The Core Common Purpose of the Health Social Care and Well-Being (HSCWB) Team is:
