The Health...SCWB Partnership, a representative group from statutory and non- statutory services, is responsible for the development of the Health...SCWB Strategy. The HSCWB Strategy is now one of the 4 major Strategies/Plans in Denbighshire. It is a statutory duty that each Local Health Board (LHB) and Local Authority (LA) jointly formulate and implement a strategy for the health and well-being of its local population. The aims of the HSCWB Strategy are to improve health, social care & well-being for the people of Denbighshire.

The HSCWB Strategy will provide us with the opportunity to further strengthen collaboration and integration between all local partners, both statutory and non-statutory. It must provide a co-ordinated response by inter-linking and cross-referencing with other major local plans and compliment the development of the Community Services Framework.

The HSCWB Strategy will be the main driver for commissioning health improvement and health and social care services!

Health, Social Care & Well-Being Strategy 2008-2011 (pdf, 197.96 KB)   ’Healthy Denbighshire’ builds upon the great work started in the first strategy covering 2005-2008. Further emphasis is now placed on partnership working, joint commissioning and collaboration to deliver better services and to support individuals and communities to improve their own health and well-being. This compliments the Community Services Framework which is about bringing services to people either in their own homes or close to their own homes in a range of community based settings. Our ‘Making A Difference’ Achievements Report will let you know just what difference the first strategy has made.
