11/01/2007 - County discharge of patients success


Denbighshire has one of the best records in Wales for the number of delayed transfers of care cases for social care reasons in the authority area.

The Council’s Social Services teams work in partnership with the health service to facilitate the discharge of people from hospital by providing a wide range of social care support in the home.

The county has consistently come in the top two positions over the last four years for having one of the lowest rates of delayed transfer cases across the country.  Figures for 06/07 show 0.72 cases per 1,000 population aged 75 or over (which equates to 7 delays which involved six people).

Once a patient is discharged from hospital, Denbighshire can provide a wide range of social care services, which can include assessments, home  care, occupational therapy and provision of equipment.

The Council also works closely with the voluntary sector and has arranged a Home from Hospital scheme, in partnership with the British Red Cross.

Neil Ayling, Denbighshire’s Head of Adult Services, said: “The low figures of delayed transfer of care cases on social care grounds in Denbighshire is testament to the excellent working partnership between the Council and its health partners to provide alternative care and support to patients, once they are in a position to be discharged.

“The emphasis is on providing practical support through a wide range of measures to support people to go home and continue to live an independent life, whilst receiving a full support service from the Council and its health partners.”
