Reablement is short-term support designed to give older people or people with disabilities the confidence to manage as many daily living tasks as possible on their own.
For example, they may need help at the moment with personal care or to prepare meals. The aim of the reablement service is to enable them to manage these tasks on their own whenever possible, rather than having other people doing them on their behalf.
This can include tasks like:
• preparing snacks and meals
• bathing
• getting dressed
• taking care of their health, and
• taking part in social activities.
The reablement service is provided by a team of professionals who work in Denbighshire Social Services. This team previously provided Denbighshire’s in-house homecare service.
How does the reablement team work?
Each reablement programme is tailored to meet individual needs. At first, a reablement team service user may need several visits every day but as they progress, the number of visits will reduce.
The length of the programme depends on individual circumstances and how the service user progresses in meeting their goals.
Are there any costs involved for the service user?
First-time reablement service users do not have to pay for a time-limited period of reablement as long as they are participating in the programme and making progress.
If they are assessed as needing a longer term care package after support from the reablement team, they may have to contribute to the costs, depending on their financial circumstances.
If you would like to find out more about reablement in general or the work of the Reablement Team contact the first contact team.
