This application form is only for parents and guardians who:

• have a child whose second birthday is between 1 September 2023 and 7 January 2024
• are eligible for the Flying Start programme
• live in either:
  • Meliden
  • East Prestatyn
  • South West Prestatyn

If your child's second birthday takes place during the next school-term, you'll be able to apply for the next round of Flying Start Childcare when it's available.
