You should only use this form if you are applying for a school uniform and equipment grant for a Looked After Child (LAC). Applications for pupils who are not LAC will be disregarded.

You can use this form to apply for a school uniform grant for one LAC at a time. If you would like to apply for a grant for more children, please complete the form as many times as required.

You must have a bank account to receive a school uniform and equipment grant.

The pupil you are applying for must be in the care of either Denbighshire County Council or another local authority, and either:
- living with foster parents
- living in a residential children's home
- living in other residential settings like schools or secure units
