The demand for both acute and community care services for older people is increasing. The Welsh Government wants to ensure that we can deliver effective integrated services to meet this growing need.
The Framework for Delivering Integrated Health and Social Care for Older People with Complex Needs sets out how this ambition will be achieved.
The framework provides a blueprint for service development and delivery, which aims to ensure that frail elderly people in Wales have a strong voice and control over the care and support they receive. It also aims to keep prevention and wellbeing at the heart of services, and to make sure that service provision is coordinated across agencies to deliver the outcomes that are important to people and their carers.

We have worked together with the other local authorities in North Wales, and Betsi Cadwaladr University Health Board to produce a statement of intent. This assesses our current arrangements against the standards set out in the framework, and sets out commitments for how we will make our services more integrated.
We will use the statement to plan our work, and to monitor our progress as we work together to deliver our commitments and improve outcomes for older people in our area.
