    Why we ask services to complete this form

    We ask each service to complete this form because it provides us with clear and consistent information to give to the customer service centre, out of hours services and other partners (who might not be familiar with our organisation or opening hours) to use over the holiday period. Some of the information may also be published on the corporate website.

    We have moved away from collecting the information through spreadsheets because that process was time-consuming and difficult to manage.

    To complete this form, you will need to:

    • tell us your details
    • let us know if your department is open or closed during the period from Monday 22 December 2025 to Monday 5 January 2026, if there are days that your department is open, we'll ask you:
      • if the department is open to the public or for staff only
      • what are the opening and closing times
      • which contact methods can be used
      • for any staff cover or out-of-hours contact details we should know
