This application form is only for parents and guardians who:

• have a child whose second birthday is between 1 September 2024 and 5 January 2025, if your child’s second birthday is not between these dates, please do not apply for childcare as you will not be eligible
• are eligible for the Full Flying Start programme or the Childcare element of the Flying Start programme

If your child's second birthday takes place during the next school-term, you'll be able to apply for the next round of Flying Start Childcare when it's available.
