How to get help from the Local Integrated Family Team (LIFT)

To get help from LIFT, a professional working with a family must contact us to arrange a collaborative communication.

What is a collaborative communication? 

A collaborative communication is a discussion with the family you work with and a LIFT team member.

We explore strengths, concerns, goals, what has already been tried and ideas for support.  Our role is to listen, understand and help. 

During the collaborative communication we begin to plan how LIFT can best support you and the family.

How to request a collaborative communication

Professionals supporting a family can contact us to request a collaborative communication.

You will then be asked to fill out Collaborative Communication form then meet with LIFT team.

LIFT can only help if we have consent from the parent or the person with parental responsibility. We cannot hold a Collaborative Communication without their consent.

Who can get help from LIFT?

We can only help professionals supporting a family if a child or young person is:

  • under 18 years old
  • living in Conwy or Denbighshire
  • displaying difficult behaviour at home, such as:
    • physical aggression
    • verbal aggression
    • property destruction
    • self-injurious behaviour
    • disruptive behaviour
  • causing severe stress to family relationships and daily life at home due to their behaviour

When we are not the right help

We cannot help if:

  • the family is open to other specialists’ teams e.g. CAMHS, Neuro developmental services, Conwy or Denbighshire therapeutic teams and CALDS etc.
  • the behaviour is not associated with a learning disability.
  • there is no evidence of significant developmental delay in two or more developmental areas.
  • the primary difficulties are not explained by a mental health needs.
  • the behaviours that challenge are exclusively in education or community settings.

LIFT is a multi-agency collaborative team, funded by North Wales Social Care and Well-being Services Improvement Collaborative.
